These Policies & Terms apply to this website and to services, trainings, classes, memberships, subscriptions, courses, digital products, and other educational offerings provided by Social Work SC, LLC, doing business as DBT Ozarks and/or DBT Hub.

By using this website, registering for an offering, creating an account, or making a purchase, you agree to these Policies & Terms and to any additional terms presented on the applicable registration or checkout page.

Website and Educational Information

The information provided on this website and through our educational programs is intended for educational and informational purposes.

Our professional trainings, continuing education programs, workshops, classes, memberships, digital products, and other educational offerings do not establish a therapist-client relationship and are not a substitute for individualized mental health treatment, medical care, legal advice, clinical supervision, or emergency services unless a service is specifically identified as such.

Mental health professionals participating in educational programs remain responsible for exercising their own professional judgment, practicing within their competence and scope of practice, and complying with the laws, ethical standards, and licensing requirements applicable to them.

Participation in an educational program does not guarantee any particular clinical, professional, business, or personal outcome.

Psychotherapy Services

Psychotherapy services offered through DBT Ozarks are separate from our educational products and professional-training offerings.

The clinical relationship is governed by the informed-consent documents, practice policies, privacy notices, financial agreements, and other documents provided to psychotherapy clients.

Nothing on this website creates a therapist-client relationship merely because a person visits the website, submits an inquiry, joins an email list, attends an educational program, or purchases a product.

This website should not be used for emergencies or urgent clinical communication. If you are experiencing a medical or psychiatric emergency, contact appropriate emergency services or go to the nearest emergency department.

Registration and Payment

Prices, payment arrangements, payment plans, subscription terms, and other purchase details are displayed on the applicable registration or checkout page.

Payment is due according to the terms presented at checkout.

You agree to provide accurate and current payment and contact information. By submitting payment information, you authorize the applicable payment processor to charge the payment method you provide according to the terms of your purchase.

We may change the price of future purchases or future subscription periods as permitted by law. Any required notice of a change to an existing recurring subscription will be provided as applicable.

Refund and Cancellation Policy

Refund and cancellation terms may vary depending on the program, event, subscription, course, or product purchased.

The refund and cancellation terms displayed on the applicable sales page, registration page, service agreement, or checkout page are incorporated into these Policies & Terms and govern that particular purchase.

When an offering is identified as non-refundable, the purchase is final except when a refund is required by applicable law.

Failure to attend a live event, complete a course, access purchased materials, or use a membership does not by itself create a right to a refund.

If we cancel an event or program entirely, we will provide information regarding any applicable refund, credit, rescheduling option, or other remedy.

If an event must be rescheduled due to circumstances outside our reasonable control, registration may be transferred to the rescheduled date rather than refunded when permitted by the terms disclosed at registration.

Recurring Subscriptions and Memberships

Certain programs are offered as recurring subscriptions or memberships.

When you purchase a recurring subscription, you authorize us and our payment processor to automatically charge your payment method at the frequency and amount disclosed at checkout until the subscription is canceled.

You are responsible for canceling before your next renewal date if you do not want the next recurring payment to be charged.

Cancellation stops future renewals. Unless otherwise stated for the particular subscription or required by law, cancellation does not result in a refund of payments already processed or a prorated refund for an existing billing period.

Access following cancellation may continue through the end of the period already paid for, depending on the specific terms of the offering.

Live Trainings, Classes, and Events

Dates, times, locations, technology requirements, attendance requirements, and other details are provided for each live program.

Participants are responsible for ensuring they have the technology, internet connection, transportation, and other resources necessary to participate.

Some live programs may be recorded for educational or future training purposes. When a program will be recorded, participants will be provided with notice. Any event-specific recording notice or participation terms will govern that recording.

We may modify an instructor, format, location, schedule, curriculum, or other non-material program detail when reasonably necessary.

Continuing Education

Some professional trainings may be approved or eligible for continuing education credit for specific professions, jurisdictions, or licensing boards.

Continuing education information will be stated on the applicable program page.

Approval by one licensing board, profession, or jurisdiction does not guarantee acceptance by another.

Unless we expressly state otherwise, participants are responsible for determining whether a program satisfies the requirements of their individual licensing board or professional organization.

Participants must complete any attendance, evaluation, assessment, or other requirements identified for the program before a continuing education certificate will be issued.

A certificate of completion or continuing education certificate documents completion of the stated educational activity. It does not independently confer a professional license, certification, specialty credential, or authorization to practice outside the participant’s existing professional scope.

Intellectual Property

Unless otherwise stated, course content, training materials, presentations, recordings, written content, workbooks, worksheets, handouts, graphics, videos, downloads, and other materials made available through our website or programs are owned by us or used by us under license.

Third-party materials remain the property of their respective owners.

Your purchase provides you with a limited, personal, non-exclusive, non-transferable license to use the materials for the purpose for which they were provided.

Unless we expressly provide written permission, you may not: copy or distribute substantial portions of our materials; share paid recordings, downloads, handouts, workbooks, or course materials with individuals who did not purchase or register for them; share your account login or course access; upload our materials to shared drives, course platforms, membership sites, social media groups, or other distribution systems; resell, sublicense, or commercially distribute our materials; record a training or course without permission; or reproduce or substantially copy our training, course, workbook, presentation, or other proprietary material for use in your own commercial program.

Some individual resources may specifically grant permission for reproduction or use with clients. When such permission is provided, that stated permission controls.

Accounts and Access

If you create an account, you are responsible for maintaining the confidentiality of your login information and for activity occurring through your account.

Access is intended for the individual purchaser unless the offer specifically provides organizational, group, or team access.

We may suspend or terminate access when we reasonably believe an account is being shared, misused, used unlawfully, or used in violation of these Policies & Terms.

Acceptable Use

You agree not to use this website, our programs, or our technology to engage in unlawful activity; interfere with the operation or security of the website or platform; attempt to obtain unauthorized access; harass other participants; distribute malicious software; or infringe another person’s intellectual-property, privacy, or other legal rights.

Privacy Policy

This section describes how we collect and use information through our public website, educational programs, registrations, purchases, email communications, and related business activities.

Clinical protected health information associated with psychotherapy services is handled separately and, when applicable, is governed by our Notice of Privacy Practices.

Information We Collect

We may collect information that you voluntarily provide, including your name, email address, telephone number, mailing address, professional credentials, licensing information, registration information, survey responses, communications, and other information submitted through forms, registrations, purchases, or customer accounts.

When you make a purchase, payment information is processed through payment-processing services integrated with our website or platform. We generally do not receive or store your complete payment-card number.

We may also automatically receive information about use of our website, such as device information, browser information, IP address, pages visited, referring pages, interaction information, and similar technical or analytics data.

Email Marketing

If you subscribe to marketing emails, you may unsubscribe at any time using the unsubscribe option included in the message or by contacting us.

Unsubscribing from marketing does not prevent us from sending communications that are necessary to administer an existing purchase, subscription, account, event registration, or other transaction.

Your Health Information. Your Rights. Our Responsibilities.

This Notice describes how health information about you may be used or disclosed, how we protect that information, and how you can access it.

Please review this Notice carefully.

This Notice applies to protected health information maintained by Social Work SC, LLC, doing business as DBT Ozarks in connection with healthcare services.

Your Rights

You have rights concerning your health information.

Get a Copy of Your Health Information

You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you.

We generally will provide the requested information or a summary within the period required by law. We may charge a reasonable, cost-based fee when permitted by law.

Ask Us to Correct Your Record

You may ask us to correct health information that you believe is incorrect or incomplete.

We may deny a request in some circumstances. If we do, we will provide an explanation in writing within the period required by law.

Request Confidential Communications

You may ask us to contact you in a particular way or at a particular location.

For example, you may ask that we use a particular telephone number, email address, or mailing address.

We will accommodate reasonable requests.

Receive a Copy of This Notice

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

Choose Someone to Act for You

If another person has legal authority to act as your personal representative, such as a legal guardian or an individual acting under an appropriate healthcare power of attorney, that person may exercise your rights as permitted by law.

We may require documentation verifying that authority before acting on the representative’s request.

File a Privacy Complaint

You may contact us if you believe your privacy rights have been violated.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

We will not retaliate against you for filing a complaint or exercising a privacy right.

Your Choices

For certain uses and disclosures, you may tell us your preferences.

When appropriate, you may tell us whether you want us to share relevant information with a family member, close friend, caregiver, or another person involved in your care or payment for your care.

If you are unable to express a preference, we may disclose information when permitted by law and when we reasonably believe the disclosure is in your best interest. We may also disclose information when necessary to prevent or lessen a serious and imminent threat to health or safety as permitted by law.

DBT Ozarks does not maintain a hospital directory.

Written Authorization

We do not sell your protected health information.

If you give us written authorization, you may revoke that authorization in writing, except to the extent we have already acted in reliance on it.

How We Typically Use or Disclose Health Information

Treatment

We may use your health information to provide, coordinate, or manage your treatment.

When permitted by law, we may communicate with other healthcare professionals involved in your care.

We may also use information when consulting or collaborating with professional colleagues as permitted by applicable law and professional ethics.

Healthcare Operations

We may use your health information to operate our practice, improve services, conduct quality activities, manage business operations, maintain appropriate records, and contact you regarding your care.

Payment

We may use or disclose health information as necessary for payment-related activities.

Because DBT Ozarks may provide self-pay services, this can include maintaining billing records, processing payments, communicating with you regarding balances, or providing documentation such as a superbill at your request.

When applicable, information may be shared with a health plan or other payer as permitted by law.

Other Uses and Disclosures Permitted or Required by Law

We may use or disclose health information without your written authorization in circumstances permitted or required by law.

These circumstances may include:

reporting suspected abuse, neglect, or domestic violence when required or permitted by law;

preventing or reducing a serious threat to health or safety;

health-oversight activities;

workers’ compensation matters;

certain law-enforcement requests;

responses to valid court or administrative orders or other legal processes as permitted by law; and disclosures necessary to comply with federal or state law.

We will limit these disclosures to what is permitted or required under the circumstances.

Missouri Confidentiality Protections

Missouri law provides confidentiality protections for information obtained by licensed social workers in the course of professional services.

We will follow applicable Missouri law when it provides privacy or confidentiality protections that are more restrictive than HIPAA.

Generally, information obtained in the professional relationship will not be disclosed without authorization except when disclosure is permitted or required by applicable federal or Missouri law, including circumstances involving certain legal requirements, abuse or neglect, safety concerns, professional consultation, or legal proceedings.

Substance Use Disorder Records

Some substance use disorder information may receive additional federal protection under 42 CFR Part 2.

To the extent that DBT Ozarks receives or maintains records that are protected by Part 2, those records will receive the protections required by federal law.

Part 2 records generally may not be used or disclosed in civil, criminal, administrative, or legislative investigations or proceedings against you based on those records unless the disclosure is authorized by you or is permitted through the court process required by federal law.

If Part 2 information would be used for fundraising communications, you will receive the notice and choices required by federal law.

Our Responsibilities

We are required by law to maintain the privacy and security of protected health information.

We are required to follow the privacy practices described in the Notice currently in effect.

We will notify affected individuals as required by law if a breach occurs that may have compromised the privacy or security of protected health information.

We will not use or disclose protected health information in a way that is not described in this Notice unless you authorize us to do so in writing or another use or disclosure is permitted or required by law.

Changes to This Notice

We may change the terms of this Notice and make the revised Notice effective for health information we already maintain as well as information we receive in the future.

If this Notice is materially revised, the current version will be made available upon request, in our office as required, and on our website.
